Anyone know if there is a qt4 plugin anywhere that allows a city, state and zipcode to be entered? Because if this is already available i would like to leverage it. Thanks

No, there is no out of box dialog for that. But you can easily assemble one by using QLineEdit with input masks and/or validation.
