Added a new action to the main tool bar but it didnot appear ??

Hello,

I am using qt creator so using the designer to create my gui,

i have added two actions and then drag them to my main toolbar.. they work fine.
but after that i added a separator and two more actions the same way ( add action then drag them to main tool bar) I can see them in the design
but when i run my application they don't show up and i even can not access them from code as they are not there at all
any idea what is going wrong ??

Make a clean build of your project and see if the problem persists.

yes i already id and the problem still there

Added after 14 minutes:

clean and build didnt solve it,
I removed the build folder and change the build directory solved the issue
