Default activeQt, add-in for SolidWorks, how?

hi, there. I've a little experience with COM.
I try to create add-in for SolidWorks via C#-example but I'am a little confused.
Add-in should be inserted in SW like a menu item.
    I cant do this stuff.
Please, help me to create a simple example with C++ and ActiveQt technology.
    I included C#-example in attachment.
    thanx-in-advance.
p/s/ sorry for grammar, cause English is not my mother tongue.
    best, Denis
