Exclamation Unable to debug simple Qt application on any of the target?

I am trying to debug Qr Decoder through Qt SDK on windows 7 and on different linux environments but I always end up with errors.
Procedure which I am following is similar to debug examples provided by Qt SDK and all examples are running fine.
Any positive result will be of great help for my project work.

    Thank you
